What to do when you are injured or contracted a disease as a result of your work?
Ensure to notify your line manager and HR immediately of the injury. The Employee in collaboration with the line manager must complete the W.CL.2 document. The completed W.CL.2 report must be taken with the employee to the doctor/hospital.
There are 4 reports that the employee needs to forward to the employer:
- First Medical Report (W.Cl 4) – initial report confirming the nature of the injury and required treatment – the doctor states either that the worker is fit to go back to work or requires time off
- Progress Medical Report (W.Cl 5) – confirming
- Final Report (W.Cl 5) – In this report, the doctor states either that the worker is fit to go back to work
- Resumption Report (W.Cl 6) – this report confirms that the employee is fit to return to work
The doctor must issue a First Medical Report. The employee needs to ensure that this report is handed or forwarded to your line manager or HR. If the employee is booked off, it is the employee’s responsibility to ensure that all doctors’ notes/ certificates are handed to their relevant line manager/HR.
If the injury continues for a long time (prolonged absence), the medical practitioner must send a Progress Medical Report (W.Cl 5) to the Commissioner. The progress report should be submitted on a monthly basis until the condition is fully stabilized. This informs the Commissioner of how long the employee is off work.
